Location and Parking: We are located in the professional building inside Advocate Lutheran General Hospital. Parking is available in both parking garages adjacent to the hospital. We do validate your parking ticket for the first 3 hours.

Insurance Accepted: Medicare, Aetna, Magellan, Humana, United Behavioral Health/United HealthCare, Advocate Healthcare, BCBS PPO

In case of an emergency, go to the nearest emergency room.

Making Appointments: Please call the office at 847-518-8490 to make an appointment. If calling after hours, the answering service will pick up the call and fax the message to the office. New patient appointments generally run 45 to 60 minutes, while existing patient appointments are generally 30 minutes long.

New Patients: We are currently accepting new patients. To make your new patient appointment, we will need your name, address, phone number, insurance information, and the reason for your visit. On the day of your visit, please bring a photo identification card and your insurance card.
